Get the 290. But, there are better cans than the ATH M50s for the money. Beyerdynamic Custom One Pro.

Soundproof case or not, the reference 290 is so loud you would still here it, wait for the non-reference ones, that's the only advice I can give you.

Thanks for the suggestion SLS but the m50s sit roughly in my price range.

I'm really thinking if i can get more for the same price as a 780 to counter the noise issue then i should be happy... Assuming the 290 is silent in idle... In gaming i might not mind too much if i have solid headphones

I understand aftermarket coolers are coming... But i own an itx rig (cm elite 120) so aftermarket cooling won't benefit me too much since there won't be much direct airflow for the card so much as the 290.

Unless someone can clarify if my rig would be fine nonetheless?

If you really want to mix ultimate performance with true silence you should get a 290, a water block, and be done with it!  ;)
