Resolution of your monitor + The resolution your xbox is set at.

 

The problem i've observed happening that looks like this, is that whenever I change display inputs on my 1440p (eg: DP PC to Raspberry Pi HDMI) monitor, all desktop icons and even the programs open, rescale themselves as if the gpu was outputting at 1080p in the meantime.

That time I saved Linus' WiFi pass from appearing on YouTube:

? No I have two monitors my Main monitor has my xbox on it. When ever I switch from pc to my xbox Skype/Discord goes on 1 Monitor. Which I don't what that to happen.

I don't have this problem. When I turn my TV off or disconnect the HDMI cable the second monitor becomes the main display and all of my programs get screwed. But as soon as I turn my TV on again everything gets into its original place. Yes, I have programs in both monitors. What Windows version are you using and what GPU are you using?

Display Fusion might ad able to do that. I'd need to check if you can set it like that. Thought your issue sounds like monitor is disconnected from pc. That wouldn't happen if you just change input. Or if I understood this correctly.
