I have had that before probably going to be bad memory module in the gpu.  I would rma it if possible.

 

At least in my case it was a bad memory module in the gpu yours could be something different but I would definetly file for a rma with the manufacturer if its EVGA gpu they are really good about there RMA's.

i dont see how it could be a dead gpu... if it was dead then it would output NOTHING. your gpu outputs the windows log on screen and your desktop.

Dead. Same thing happened to my GTX 460, baked it, nothing happened. In your case, it might work. Give it a shot.
