Hardware failure causing windows to stop working correctly

Hey,so i was playing fallout 4 and my game froze and my display stoped recieving.This is the third time this has happened.My CPU and Gpu passed burn in tests (prime 95 and furmark). Im running an r9 380 gaming 4gb (using 1000mhz OC setting in msi gaming app) and an i5 5690k (stock).I checked the reliability monitor and got this (screenshot) and when i viewed the technical details of the hardware error i got this-
