I would say rear as the chances are there is already a fan there to assist. If you set the cooler up in a push config, the fan in the rear might be able to assist in pull.

Depend on case and mobo (if there is empty slot between cooler and GPU or not). I had in previous setup 1 rear and 1 top exhaust So pretty much equal, though top was 140mm. In that situation I found that it doesn't matter for cooling performance which orientation you use. So if you have only rear exhaust, use default orientation. With top exhausts you can use horizontal as well.
