Leave Threaded Optimization on Auto. You should have more than two settings for V-Sync. You should have On, Off, Adaptive V-Sync, and Adaptive V-Sync(half refresh). Generally speaking, I use Adaptive V-Sync unless I encounter issues which I correct using the Program Settings tab. The main tab I use is Global. If you plan on playing anything like NFS Rivals, Use Program Settings to set V-Sync to on because Adaptive V-Sync will disable V-Sync causing tearing.

i suggest leaving most of the nvidia control panel on default.

vSync is personal preference. i prefer vSync enabled, and on *some* games (like sims 3) that are heavy to run, and dont benefit from high fps, i set it to "half refresh rate"
