Hi, I've been having problems with my 290 and gigabyte say they cannot find a problem with it.

There is enough power going to it, and I've tried it in multiple builds with different monitors and cables (DP, HDMI, DVI-D), OS (Win7, Win8/8.1, Win10, ubuntu), and different drivers and versions of catalyst. It causes crashes, and lots of visible anomalies.

Any idea on the specific causes or possible fixes?

Looks really obviously like a dead card :l are you getting bluescreens or anything? or just the visual anomalies, which COULD have something to do with drivers, but seeing as you accounted for that I really can't say anything more than it must be a hardware problem if you're experiencing the same thing with different builds and different monitors.

Which revision is it?
The original model was reported to have issues with vram and all that stuff.
Revision 2 was fine.
