Is the airflow bad? If you have something like a 290 would it run hotter than something like a 450D or Arc Midi R2?

It's much more restricted than something like a Corsair 450D or Cooler Master N600, the difference is by a couple °C and can be minimized a lot if you get proper pressure optimized fans, something like 3 AP fans from silverstone on the front will get you decent airflow going on, Noctuas,  NoiseBlocker fans, Gentle Typhoons, and similar options have enough strength to fit air through those small holes. I wouldn't do a R9 290X crossfire in that case though.
