You'd first need to assign a static IP to the LAN device you'd like to open the ports for. You'd then put that IP into the LAN IP box, followed by entering the port numbers into the WAN and LAN port number fields, one port at a time. Choose the correct protocol required by the application / service you're looking to open ports for, and voila! That being said, some modem/routers make opening ports far more difficult than need be, so your mileage may vary.
