Samsung 1tb SSD

I just got back an RMA "replacement" ssd from samsung.  I have had issues where anytime I try installing a larger game it fails;  I tried r6 from steam and COD MW from battlenet, both over 120gb files.  The drive usage is stuck at 100 percent at all times. 
I made sure to format it correctly in disk manager; I made sure to update firmware.  I removed the 2 other working SSD's from the system and used the port, and cables from a working one to test.  I still have corrupted files or failed installs.   The disk still shows at 100 percent usage when not even in use. 
I am at a complete loss.

PLEASE someone make sense of this .

I was able to install some smaller games and they ran fine.  But nothing larger works.

Keep trying to sign this stupid screen but it wont work...
