Is my mouse dying?

I have a DA 2013 and here is the problem.

 

When its plugged into the USB 2.0 port on the back(and i tried all the USB ports on the back) and I restart the PC after the PC restarts the mouse doesnt turn on. I have to keep unpluggin and plugging it in from the back port like 8 times to get it to turn on.

 

When Its plugged into the USB 2.0 port on the front the mouse doesnt turn off when I turn off my PC.

 

 

what the hell is going on?

CPU: AMD FX 8350  Motherboard: Gigabyte 990XA UD3  GPU: Gigabyte GTX 770 Windforce 2GB  HDD: WD Green 2TB SSD:  Corsair Force GT 120GB SSD RAM: Corsair 8GB(2X4) PSU: CoolerMaster G650M

Sounds more like a USB port issue. Test it with another mouse or USB stick.
