So I reinstalled GTA 5 after a long time of not playing to try the new indy car racing, but I can't seem to complete a single race because my game keeps crashing. Whenever it crashes, both my monitors will go black, then there's a quick flash of static-like artifacting, then the error message pops up which reads "ERR_GFX_D3D_INIT Failed initialization. Please reboot and restart the game. Try reinstalling the game if this persists." I'll leave a picture of the error message attached to this post.

 

I've never gotten this error message before and have never seen it before. Here's what I've tried: I bought a new Samsung 860 EVO 1TB SSD less than a week ago so I was worried it was my new SSD. I downloaded Samsung Magician and updated firmware. This didn't work. I then moved GTA 5 to my operating system's SSD, but the crashing persisted with the same error message. But at least I confirmed it wasn't my new SSD. I also tried rolling back my GPU drivers (My drivers were up to date) but this also did not work. I've also experienced some crashing in Call of Duty: Modern Warfare, but it's less frequent than GTA 5, so I'm not sure if it's related or not. Although at this point, GTA 5 is a guaranteed crash after about 10-15 minutes right now, and Modern Warfare is not. I've also been playing Halo on PC but neither Halo 1 nor Halo Reach have crashed. CPU usage is normal & temperatures are fine across the board.

 

I'm not sure what else to try, so I'm open to all suggestions. Here are my specs:
