If you can't find either info it's never going to be usable again unless you go to Apple with a proof of purchase.

Even with that, can apple get into it? Personally I am not sure how this works.

After a wipe yes they can bypass the activation lock.

 

For OP the best is to try to recover the iCloud account with a password reset if needed.
