stuck at 405 mhz

i have a gigabyte 750 ti that i overclock normally and i thought i reached a stable overclock but now i cant get it to run over 405 mhz this has happened before and i reinstalled my drivers but after a while it went back to sticking at 405 should i reinstall again or do something different?

restart PC. don't shutdown and turn on RESTART.

Your GPU went into Save Mode.

how do i prevent it from going into save mode?
