Im having a few issues with my 960 when trying to overclock, I can comfortable hit +200 on the core and +390 on the memory with stock voltage, however any increase in voltage causes it to crash.

Im also having some issues when turning on my PC and nothing coming up on the screen, my pc boots into windows as the RGB profiles for my mouse load once windows is running and sound plays, this is very annoying as it doesn't always turn on for a few attempts. Do I just have a bad card or is it something else?

Ive managed to get it to +350 on the core and even something like +500 on the memory but it keeps causing driver crashes. I got halfway thru a benchmark before it did that

it's not stable therefore the OC is not good

I have a feeling thats something to do with stock voltage tho no? Also adding voltage caused it to crash even without an OC

Man, +200 on that card? Could you GPU-z your clocks? Coz +110 on Core and +170 on memory caused crashes on my card..

Voltage controller could be junk.

Im putting it down to 2 things, the card sucks, or my psu sucks. Also could either of those be the reasons that it fails to run sometimes on startup?

....

If you can OC it as much as you can, obviously the core in the card is pretty good and the PSU can supply it clean power without it crashing. If it crashes when you try to change the voltage and in no other situation then the voltage controller might just be junk which would also cause problems on startup.

Like I said earlier, I can get it to run fine for a few mins with an even higher clock, adding voltage causes drivers to crash, ill test it again once I get my 650x and see if that makes a difference.
