Strange Issue with Windows 10. I think?

Same problem as you have, the first comment has the solution.

So here is my current problem.

 

This problem has been happening every time I finish playing Rainbow Six Siege. After playing I close Uplay and disconnect my Xbox controller and then my mouse makes clicking noises and moves in super slow-mo.

 

If I try to Ctrl+ alt + Del I get a black screen and have to hard reset the computer. 

 

I also have this issue sometimes when I try to change the Gpu fan speed and it hit apply, my whole screen goes black and I have to hard reset.

 

Is this possibly a Windows 10 issue or could this be completely something else?

Sounds like something else, but still software wise. I would reinstall GPU drivers using DDU, check Device Manager and look for anything wrong, perhaps the Event Viewer for any critical errors.

I always use DDU when I reinstall my GPU Driver.

 

What I noticed is when I disconnect my wired xbox one controller after playing, that is when the issue occurs.

 

If I don't use the controller the issue doesn't occur.

Reinstall the Xbox 360 drivers.

Yes but you can remove them. Go into Device Manager and see if there is anything on it, try the 'revert to a previous driver' or whatever it's called.
