Recommended Upgrades?

This is the current build, I built it back in 2014. The reason I'm thinking about upgrades is because I'm finally getting around to The Witcher 3 and on medium graphics the game is choppy. Budget is around $400. Thoughts?

it sounds like you're running out of VRAM.

go for a 4gb 380 or 380x

used 290s or 290x's are also great options

This upgrade includes new motherboard, cpu, and gpu. It costs $410 ($390 after rebate) for the 3 new parts.
