Is my SSD dead or broken?

Hi all!
I have a Intel SSD 120GB it was my main boot device and couple of days ago my win died with the error 0xc000000e 
now i checked the web for a solution nothing helped but a reinstall problem is every time i installed win10 on the SSD i would show the error again 
i switched Sata ports and cables! Bios never sees the SSD after the error only after i install win10 and go to storage management and Format the SSD from there.. 
what is wrong with my SSD?
