Black ops 3 setup!

I just bought COD Black Ops 3. Using nvidia surround you can make split screen go onto two different monitors. I really want to do this but need help finding what I need. I want to keep my main 1920x1080 monitor plugged into the hdmi port on my GTX 760. That leaves a display port, a dvi-d, and a dvi-i. I have two old 1024x768 vga monitors that I want to do the splitscreen on. What do I need to make this setup work?

All the monitors must be the same resolution for surround to work.
