Problems installing Windows

I've tried numerous times to install off of my windows 7 disk and it gave me a code 5.. Then I tried formatting my USB to UEFI BIOS. I've been trying to install my windows for the past 3 or 4 hours; this is so frustrating. Everytime I would plug in my USB it would say "Remove disks or other media. Press any key to restart." Then after I hit a key it would say "Reboot and select proper boot device or insert Boot media in selected boot device and press a key"

Anything you guys would suggest that would be helpful.. Thank you.

Did you copy the windows to your USB correctly? If you didn't, it won't recognize the USB Boot when you boot up the system.

Well I have a disk too and I get a code 5. It would be much easier installing it from the disk but I get the code 5 error which I don't understand why..

First off what program are you using to build the windows 7 usb? Second what kind of hardware are you running on and what are your current BIOS/UEFI settings?

If you're using an older ASrock or MSI motherboard I found this article http://www.unawave.de/windows-7-tipps/code5-error.html?lang=EN which goes into detail about how to fix the code 5 error.
