ASUS ROG Swift - double image

Specs are in my signature. I bought this monitor back in October 2014 and have been using it ever since. However, a few weeks ago the monitor would suddenly start acting up for no reason. Everything suddenly looks blurry. If I turn the monitor off and on again, it says "no signal" and I have to restart my PC for it to work again. This can happen at any time; when watching a movie, playing games, browsing through Facebook, etc... There's no pattern to it at all. It would happen on very rarely but every time it happens, it seems there's a shorter time between it happening again. Here's what it looks like:
