Screen randomly goes black for 2 seconds when gaming

My friend's had an Hp monitor for monitor for months now 

 

Monitor: really large screen , I'm guessing a high-end gaming one.

 

Video card: GTX 550

 

Problem: Screen Goes black every few seconds.

 

Usually Happens when:gaming above 50 hertz or when gaming at above 50 without V-sync

 

Tried:

Decreasing brightness

Changing windows default hertz to 50(which doesn't work)

hOW CAN YOU TELL if its the driver crashing?

Are the GPU drivers correctly installed?

What kind of cable is used?

Are the connectors well secured in place?

Also please try to get the model name.
