I am not 100% sure that this an issue of my video card but from the looks of things it is.
I have a dual monitor setup with a samsung 1680x1050 and a HP 1280x1024.

The samsung monitor is running of my hdmi from my video card to the dvi on the monitor (with a male to male cable adapter thing)

The HP has a DVI to VGA adapter.

and when im unpluging the hdmi from my card this is happening

post-3316-0-58794400-1372437779_thumb.jp :

So im guessing it only happens to the primary monitor.

I have gotten this problem twice first time after a while it was fixed and the second time i restarted my pc.
I dont think some program was doing it cause i wasnt doing anything special and after it happened i closed everything.

Here are my specs:

Intel i5 3570

16GB RAM corsair vengence

Gygabyte Z77X-D3H

Saphire HD 7770 Ghz Editon
Samsung 830 256GB
WD blue 1TB
CX 500 watt corsair psu

Sorry for the image quality :)

Does it happen when only using a single display?

What happens when you use your HDMI connected monitor only?

Have you attempted to reinstall the drivers? Any overclocking done?

The card may be malfunctioning because of a hardware issue.
