what does port mean?

They mean that the game was originally designed for console, and then afterwards the developers have copied it over to PC. 

 

Usually this is worse because it means that the game has been designed more for console and won't necessarily take full advantage of PC hardware.

it means its not an new title, but instead one that is now available on a new platform

Basically a copy of the console version slightly optimized to run on the pc
