They intake with the two fans and outtake primarily out the sides of the card. Some air does leak out the case though, just not as much as a reference cooler. 

Alright because yeah, I put my hand next to it (near backplate) and there is really no air, so I guess it's exhausting all out the back, which is great since I'm using a 250d.

I know and a lot of people also do but I'm just pointing out that some reference coolers are different :P Heck even the 295x2 doesn't have that kind of design or the 7990 or 690

I believe the 690 does blow out the back, doesn't it? From what I've seen, it isn't open on the sides.

But I don't feel any heat inside or near the GPU. Only at back and a bit in front.

please post a picture of the exact card you have. I'd have to see it to know exactly what's going on and also, is this while you're using the card or when it's not being used (e.g gaming or just on desktop?)

Gaming, I reached 68 degrees max on arma 3. It's the GTX 770 from ASUS, the DCii cooler type.

that's really weird, that cooler mostly blows air back into the case not out. Not sure what's going on man sorry... My best guess is that it's mostly blowing it onto the mobo and it's going up from there so you can't feel it.
