HDD Reallocation Sector Count error , can it be fixed?

My HDD has become very slow lately and most of the games on it wont even start , so i ran hdd scan and this are results.

Also usage is always at 100% , average response time is always spiking over 1000 (sometimes even over 7000) 

Can it be fixed or not? 

(i use hdd only as storage , windows is installed on ssd which is working properly)

Nope. It's a sign the drive is going bad. Because it's moving/moved sectors it can't write to anymore to a another portion of the drive. You better off buying an new drive.
