If you're talking about the option in the installer, it does the best it can at completely removing and reinstalling the driver.  DDU will also do this for you, and works rather well.  Download DDU and the newest driver for your card, remove the driver with DDU and reinstall with the new installer, this is the cleanest way to install the driver and ensure nothing is left behind.  

 

That said, unless you have some sort of driver issue that's causing problems, I wouldnt bother with all that, especially if you have profiles for games or custom settings/resolutions.

But if I don't have any custom profiles is it okay to do a clean install?
