Video card sometimes disconnects by it self

So the problem is that sometimes I hear win 10 disconnect sound and boom, not every time but often pc freezes( have to manually restart whit button cuz nothing else works), but sometimes it doesn't freezes but if I go in some game I have low fps. 

What could be the problem? 

 

Have this problem since build this pc (4 months a go) 

 

Nothing is overclocked!!! 

 

Rest system: CPU ryzen 5 1600af

MB asrock b450 hdv r4

RAM g. Skill 16 GB ram kit 2666mhz

PSU Fps group 700w

STORAGE Kingston ssd 240 GB, segate barracuda 2tb(games), old Samsung 1 TB hdd(files)

Clear the cmos 

Update the bios 

Run memtest86 to make sure 

Ddu the drivers 

Check temps

Have reinstalled drivers whit Ddu already, temps are OK, bios is newest, will try memtest86 later

 

And psu is about 3-4 years old

 

Made a mistake its 700w

Fsp si series 700w 50arn 88% efficiency

Can you if possible try a different PSU

OK I'll try

 

And have done many stress tests(gpu and cpu) it's seems stable psu

 

 

Thing mostly happens after it wakes up from sleep, but maybe once or twice a week
