PC randomly loses input signal

The problem started occuring about 4 months after me building the PC and never having any problems. It lost input signal and its happing since. Its sometimes working fine for days and sometimes it happens evertime I start the PC. There is no patern in when the problem occurs but there are some different "versions" of the signal loss.

starting PC --> monitor isnt getting any input and the VGA EZ debug-LED is on ( indicates GPU is not detected or fail )
starting PC --> monitor turns on and I can see the boot screen --> loses input signal and the EZ debug-LED stays off
starting PC --> everythings working until I log into windwos --> loses input signal and the EZ debug-LED stays off
starting PC --> everythings working --> randomly losing input signal and the EZ debug-LED stays off (sometimes the audio is still running and sometimes its cut off after a view seconds)

A restart of the PC sometimes fixes this but most of the time I have to resart multiple times and wait minutes with the PSU off.

My tries fixing it:

checking DP cable and then trying DVI
checking monitor with other device (PS3)
testing an other monitor
checking tempretures of GPU and CPU
rebuilding the whole PC
complete reset of the system (wipe the ssd and installing everything new, BIOS/CMOS reset and BIOS update to 7A38v98)
trying older drivers
