Vsync only show its benefits when fps is larger than refresh rate, so you should just turn it off.

What @fasauceome told me when I asked kinda the same question, is that if you get an fps lower than 144, but is consistent (example: it hovers at 100 and stays there) then it should stay somewhat smooth. I get less than 144fps with my 144hz monitor but it still stays fairly smooth because my fps is consistent.
