BuyVM, TACServers, RamNode, Hostigation, and QuadraNet are my go-to right now for VPSs (even though I mostly use my own company).

Dacentec, Kimsufi/OVH, and Online.net are where I go for dedicated servers.

WideOpenWest, QuadraNet, Handy Networks, COLO@, Constant, and CNServers for colocation.

Other VPS providers I use:

Crissic (purchased by QuadraNet)
DeepNetSolutions
Hostodo
ImpactVPS
Inception Hosting
NodeServ
QuadHost
RansomIT
ServerHub
VirtWire
ZappieHost
