10GBase-T can be used with standard RJ45 faceplate termination aslong as you are using cat6a+ for the cabling, that is however limited to 10G at a 100m distance.   Requires a 10GBase-T capable switch though, as far as I know there isn't such a thing as a SFP+ or QSFP faceplate nor would the enterprise industry ever have a need for one.

If you want to do fiber you have two options with a standard wallplate, either an LC fiber coupler or an MTP/MPO coupler like this:

Then you can do either a QSFP BiDi optic and standard OM4 LC-LC cable

or

QSFP MM or SM optic with MTP/MPO cables.

Just a standard LC-LC fiber coupler installed in a wallplate is what I use for 10G fiber :)

Pretty much as above, QSFP is the transceiver interface, not the physical interface of the cable.

 

Standard LC plates would still be used most commonly, or MTP/MPO, although I haven't seen those broken out outside of a local rack.
