SD card formatted as internal storage

Hi everyone, so I hope someone can help. 

I got myself a 16GB SD Card and formatted it as internal storage on my Android device, but yesterday I took I removed my SD Card and formatted my device and when I inserted the SD Card I couldn't access the data on it. 

 

My PC can't even read the SD card.

 

Is there anyway I can get my stuff back from the memory card?

Sounds like the SD card might've been encrypted by the phone. Either that, or it's corrupted now and needs to be reformatted.
