Video/audio switch

I'm setting up a gaming desk with Xbox One/ PS4 and PC. I'm looking for a simple solution like a switch that can take inputs from HDMI on consoles and Display port on PC and Output Display port and Optical audio for a soundbar and to a gaming monitor. When using my gaming headset it will be plugged directly into the PC or console controllers.

 

Any suggestions for maximum performance from all 3 types of input. I've been holding off choosing a monitor until I sort this out.
