I've been having this issue ever since Windows updated to 1803 against my will. This happens with any game. Sometimes, the game tabs out on input. In Witcher 3, I've observed it with the ESC button, scrolling, and even a left-click. In MGS:GZ, it just happened on its own without any input. Same with Dark Souls II. It seems like something is taking priority over it, but what? I've tried without MSI Afterburner open, and GFE is not installed. Ansel is also disabled.

To note: this only happens once every game start. If I tab back in after the tab out, it doesn't happen again until I exit the game and restart it.

Game leaving out, is because they lose focus. This is because something is taking focus. It could be a driver bug, or a software that takes focus, and when you click the game notices that it never had focused and minimizes.

So,

  • Make sure ALL your drivers are fully updated
  • Uninstall mouse/keyboard driver/software
  • Disable all startup programs (don't skip Task Scheduler startup tasks)
  • Your A/V or other security software might be at fault

All drivers are fully updated aside from M/KB. I'm using a generic keyboard and a Corsair M40 mouse, both are using the default Windows drivers. Only Realtek Audio Manager and Windows Defender notification icon are on at startup.

I also use the free version of MBAM, so it's not constantly checking. Windows Defender was left as default.

BTW, I tried switching to borderless Windowed in Witcher 3, and that solved it, but I do get some odd stutter in there, so I switched back to fullscreen. Thought that would help with a solution.

Made some progress on the issue. I can confirm that it's something with Steam.

I launched Witcher 3 through its .exe, since it's DRM-free, Steam didn't open or inject the overlay into it as well. No tab out at all. Played my CD copy of Fallout NV, no tabbing either. So it appears to happen only with Steam games.
