PC Crashes and freezes randomly when gaming !

Hi !
I have had an issue for awhile now and have been unable to solve it.Sometime when i gaming ,my PC was crashed and monitor was lost signal,when i restart it and right after that a PC was froze (i guess so cause when i press caps lock or num lock,the led on keyboard was not changed) and a monitor still without signal.I can't power off a pc when i hold power button on computer case until i disconnected the power and reconnected it,i can power on a pc and it run normally.

My Specs:
Type : Desktop.
OS : Windown 10-64 bit.
Motherboard : Asus Rog Strix B250F.
CPU : Intel Core i5 7400.
VGA : Asus Dual rx 460 O2G.
RAM : Gskill 8 GB.
HD : HDD Toshiba 1TB.
PSU : Seasonic S12II 520w.

Temperature :
- Idle :
+ CPU : 39 °C 
+ GPU : 33 °C 
+ HD : 35 °C 
- Gaming :
+ CPU : 55-60 °C 
+ GPU : 45-50 °C 
+ HD : 38 - 39 °C 

Please help !!! I built this whole system 6 month ago and I feel so sad. :'(

The monitor isn't causing the issue.

Maybe the GPU is? Check all cable connections. Completely uninstall and reinstall drivers.

Do some stability tests.

 

When did it start happening? You did something or it just started happening? 

You overclocked the GPU?

The last time it happened was one month ago and it has happened 3 times since i built pc (in late 4-2017)
I didn't overclocked Gpu, my rx 460 is rx 460 o2g version, it was default overclocked  by asus.
I will reinstall drivers of gpu.
