R9 380 overheating

Ok so I bought a used R9 380 put it on my pc and the temps were 99ºC, I then bought a new pc case (the define S from fractal design) because maybe it could be the cooling since I had a bad case but the temps are still 99ºC, so now I don't know if the problem is still the case and should I buy more fans or is it the gpu.
So what can i do?

idk if specs are importante but:

cpu - ryzen r5 1600

gpu- r9 380

case- Define S (with default fans (2))

Motherboard- MSI b350 Tomahawk

Cooler - stock 

Ram - 16GB of Vengeance lpx

I'm currently underclocking and undervolt  it to 900Mhz and it runs at about 72ºC

I will replace the thermal compound tomorrow since some of you suggested it

Either the temp reading is bad, or the TIM is cracked, either way replace it. I have an R9 380X and even overvolted to all hell on water it never went over 60¨C

Download the program. open it, third graph on the right is GPU Usage. Just do the old "Ctrl+Prtsc" and send the screen shot our way.

One time I did get an erroneous reading of over 256°C for a second, I was like WTF then oh what the hell. The card was heavily volt modded when it happened and had a AIO on it, running close to 1210 MHz. So the temperature sensor definitely can make mistakes, however I'd say it's the thermal compound.

So it looks to be running at 69 C while at 100 ish % load

Those temps look perfectly acceptable, what programme said it was running at 99°C?

That is.... very weird, I've never seen such a massive jump in temp for such a small frequency increase. What kind of cooler and model of card is it? Either way change the thermal compound, that should fix that.

Yikes! Alright then! I'll suggest what everyone else said, which is replacing the Thermal Paste. (I'd suggest Arctic MX-4)

Wow, I assumed it was a reference card, yeah the thermal paste must be bad or a heatpipe is punctured, possible multiple ones, although it should still cool. Just replace the compound and it should go down to the 50°C's at 100% fan speed.
