So there is this Youtube channel I watch and sometimes they do this thing where they will make their mouth really big or eyes small for comedic effect but the blending and tracking is extremely well done. So from watching it I think they just take a mask from the orginal video and feather it like crazy so it blends into their skin. Then they either use the AE camera tracking to put it on or just go based off of it being the persons head so it moves the same. Watch the videos for the first couple seconds.

that opening scene would not be that hard.  Just duplicate the main video layer, mask it so only the mouth is visible, and then motion track it onto the eyes.
