I'm gaming at 1920 x 1080 resolution on Windows 10, most of my graphics settings on medium to high for every game I play, looking for 60FPS a second (because I mostly play fighting games.) I'm currently running a AMD Radeon R9 380 series.

The issue I'm having, and only started about a week ago is that I'll be gaming and my screen will freeze and go black, and shut my game down, or the sound will start skipping, leading to a completely green screen and force me to shut down my PC altogether. Sometimes I get a warning, like a thin green line across my screen and I quickly alt+f4 my game. I uninstalled my graphics drivers and reinstalled them to no avail. First it was only on Tekken 7, but now it's happened on Heroes of the Storm and Diablo 3 as well, which I've never had issues with in the past.

When it happens, I hear my fans go on overdrive, but I don't think overheating is necessarily an issue, I just installed a Noctua NH-L12 120mm & 92mm SSO Bearing PWM Fans CPU Cooler on my processor, and upgraded all the interiors.
I'm running an intel i7-6700k CPU @ 4GHz and 16 GB (4x4) of RAM. 

I just don't know if I need a new GPU or what.

Okay. Well Corsair released 2 versions. One was the green. It was not the best PSU, but the grey one is good. Anyway. What are the temps of the CPU/GPU and the usage when gaming?

I'm not sure.  I just finished up a complete wipe of my system and reinstall.  I'll reinstall a game and something to check the temperature and write back as soon as I have it.

Okay, just as a quick reference, according to HWMonitor, my CPU temps while just downloading the Blizzard App:

Core Package: 143 F (Value), 73 F (min), and 145 F (max)

While doing nothing:

Core Package: 73 F

GPU: 84 F, and 84F Min 93F max

 

Edit: and I don't know how much of a difference it makes, but I'm using a Corsair Carbide Series CC-9011070-WW Black Steel Micro ATX / Mini-ITX Air 240 High Airflow MicroATX PC Case

Installing Heroes of the Storm now since it was the last game I played that froze my computer.  As of 2 weeks ago I hadn't had this issue, and I don't really understand why it suddenly started.

I believe they're all connected correctly, my GPU only has slots for 2x 6 pins, and both are hooked into.

Okay, so while gaming, my CPU stayed at about a value of 134F with a max of 158F.  My GPU though kept growing as the game continued, going from 104 to 138, 143, 150, 154, 179, and finally capped at 181F in value and max, before shutting the game down.
