Sound Card Configuration

So I'm setting up a PC with a playout system (PlayIt Live) to be specific, this is quite nice free software and is quite configurable, I am configuring it to have two playout carts which will drag audio from a log, and also a third standalone player which will be used for things like pre-fading, playing things outside the log and so on. I will also have a hotkey panel.

Now the problem is configuring all this, I have three fader channels which I need to route to through either Balanced XLR or Jack on a mixer.

 

So two channels will be for the playlist carts which when playing alternating, so cart A goes to fader A and cart B goes to fader B, songs play in an A-B-A-B-A-B fashion.

 

The third channel will need the standalone player and hotkeys

 

Now the problem is getting all of this to route properly, this is all going to be happening on a HP DC5700 desktop PC, at present this has only its default Realtek audio which can't do this so what I need to know is whats the best way to get three separate audio outputs out the same PC so I can configure the software as described above? I realize I will probably need some extra hardware to do this so will be happy to take relatively cost effective suggestions, another thing to consider is this is a SFF PC so needs low profile internal cards.

 

Anyone know any sound cards that might be able to do this for me?
