Which one is the bottlenecker ?

Your issue doesn't sound like a bottleneck, it's probably your GPU running out of VRAM (the 750Ti only has 2GB of VRAM) or you are running out of system RAM. 

I'm currently using an i5 4460 with

a Palit 750ti StormX Dual (specifically that card because the boost from FE is major). I received a lot of stutters in games like GTA and Shadow of Mordor so I want to check the usage and identify the bottlenecker but I saw both the usage of CPU and GPU was 100%. The games are in full screen and least as possible background tasks. I know the setup is a bit weak for big AAA titles but I'm upgrading in a year or two. How can I identify the bottlenecker?

The sensor from afterburner reads approximately around 1900MB used, i thought the vram problems only happen when it exceeds

in gta you can sett the seting so that you dont use all of the Vram, i got the palit gtx 750 ti StormX oc (only has one fan) and with the settings set a specific way i get a stable 60fps. so maybe it is your CPU since gta is more CPU heavy

That is very close to the limit, I think you should start looking for a new GPU with at least 4GB.

it is close, but nothing to be a bottleneck, mine is running at 1997MB

yeah, that might be the problem. try it without and see if there is a big difference, but you probably knew that that's what you need to do now XD.
