Microsoft Access help

I need to resolve a Many to Many relationship in mu database work. I need to solve this for a doctors surgery (That's what we call it in the UK) 

 

The many to many relationship I have is 

 

Patients------------------Appointment

 

How would I solve this? Can't think  

 

EDIT- I'm thinking I could use a Doctor or Unique Patient ID
