Audio Pans When Increasing Volume

As I increase the volume while using my AKG K240 mkii's the audio pans mostly over to the left channel and the volume increases dramatically with very little adjustment. I tried swapping the cables only to get the same result. I would assume this is due to a damaged wire inside the headphone assembly...is this the case?

Actually I hadn't thought to test it with any other drivers cuz the AKG's are the only headphones I own, but I just now dug out some old earbuds and the same thing is happening there...sorta. The planning is barely noticeable, but when I pass the 50% volume mark the volume jumps and the earbuds immediately begin to clip. Should this be an issue with the phone, any idea what it is?
