I just bought the Asus RT-AC66U and it has been fantastic. Super fast AC and it has two USB ports. I don't have any personal experience with a home switch though, so I'll leave that recommendation to someone else. :P

Edit: The RT-AC68U is going to release in two days, so maybe wait and see what happens to prices after that.

With that kind of budget you can afford a really good router but I am not so sure about that 12 port switch. My guess is that one will cost maybe ~250 dollars alone. What kind of modem do you need? ADSL? Cable? If you already got one today (probably built into the router your ISP gave you) then I think you should use that instead of buying a new one.

If you want 802.11ac then you should take a look at the Asus RT-AC66U (189 USD). It seems like the most popular 802.11ac router on the market right now. The Linksys AC1900 (200 USD) seems good as well (slightly faster than the AC66U, don't mind the negative reviews because they are written by idiots). If you are OK with a single USB port then you can get the Netgear R6200, which is a 1200Mbps 802.11ac router for "only" 135 USD.

What are you going to use the switch for? You are probably better of getting something like two 8 port switches, since it might offer better performance in certain scenarios, and it will most likely be a lot cheaper (will probably cost something like 100 dollars for both).
