SSD I/O Device Error

I recently added a SSD drive that i wasn't using to my system at first the disk was showing that it was a locked disk but i moved it to a  different SATA port and that fixed that issue. but now when i try to initialize the disk it is saying it has a I/O device error as shown in the first image attached also SSD make and model in second image. I don't know how to fix this and i hope the drive isn't dead.

 

please can anyone help me to fix the drive.
