SSD Dying?

Hi recently my computer has been acting very odd with numerous crashes within apps (CTD crashes) or my computer just looks up and i have to force a shut down. Ive looked at these crashes in event viewer and I have this error.

The IO operation at logical block address 0x1c7400 for Disk 0 (PDO name: \Device\0000002e) failed due to a hardware error.

Is this a hardware issue? My boot device is a PNY 120gb CS1100. It has been written too alot because its my photoshop scratch disk and is what i use to store my current Photoshop projects. It also has Windows 10 (64 bit) on it.

Alright thanks says ssd is 100% healthy.

Alright now i get the BSOD- unexpected store exception. at this point since i might just do a clean install since i can get into windows and get any important files off the os drive.
