Every 10 or 20 minutes, my screen will look like this until I restart it (Not my image, I found it online because I cannot be fucked taking it myself If I can find the exact same problem online) I'm running onboard at the moment while I wait for my 390 to get shipped, I dunno if it's related or anything but I recently uninstalled my old AMD drivers.

Try another monitor if possible. If it looks fine on another monitor, your monitor is dying.

Cable or input issue? Try different cables/inputs. 

My monitor is DVI-D only.

Don't have another monitor on me, I'm 100% sure it's the onboard graphics, because it's more common when my CPU is underload.

What cooler are you using?

I would try re-applying thermal paste.

I've tried that, I'm running a CoolerMaster Sedion 120v Plus with a push pull config.

I have had this problem as well. If there is a high temp spike on the GPU core, you might the blocks and lines. Mine did this because of my overclock on the CPU side and not the GPU side. Adjusting the voltage on my overclock fixed it. But I'm not sure if this is the same thing for you. Just sharing my experience.
