What speaker set do you have? I have a Dell 5650 5.1 Surround speaker set, had that happen when the volume control died, now I'm locked at max vol. Could be something similar.

 

Edit: I don't use integrated anymore, so not sure what setting it could be. Also make sure you don't have any malicious software or anything conflicting software.

Click the speaker icon in the Notification area, then click the Mixer link. Check to see if t0wer's suggestion is the problem.
