Would this fan set up be efficient?

I'm assuming you have some front intake fans, and top intake fans.  Yes this will work, and give you positive air pressure.  Should work very well.

Would a push/pull rad used as the only exhaust in the back be efficient. Also with a top intake 200mm

As in just a 200mm intake on the top, and a 120mm push/pull rad in the rear mount for an exhaust??

I'd suggest you get at least one 120/140mm fan in the front.

A decet setup without having like 9 fans in the case would be to have two 1400mm (120mm would be fine) fans in the front for intake, a 120/140mm in the top and a 120mm in the rear for exhausting.

I recon that'd be fine, a 120mm fan in the front and bottom for intake, with the rad on the rear mounting position and a 200mm fan on the top as exhausts. 

That would cause negative air pressure, but I have no idea if that's recommended (as opposed to neutral or positive) or not.
