GPU upgrade ?

I have a dell optiplex 745 (got it from a friend for free) it has a gt7950 it it artifacts a lot. I'm thinking the card is ready to die even though it does greatttt in games for being 8-9 years old but now I was looking into getting r7 250e from xfx and a cm 430w good mild upgrades or no ?
